What this policy covers

This policy explains what happens to information you enter on this website. It is not our HIPAA Notice of Privacy Practices — that is a separate document, and clients receive it when care begins.

What we collect, and when

We do not ask for anything until you choose to fill something in. Specifically:

• The eligibility check runs entirely in your browser and stores nothing. Your answers are only sent to us if you ask us to call you back — and then only together with the contact details you type.
• The waiver application collects the information the Medicaid waiver requires: contact and Medicaid details, the providers involved, medical history, and how much help is needed day to day. It also records your signature on the authorization.
• The chat button collects your name, a phone number or email, and your question.
• The referral form collects the referrer's details and the prospective client's details.
• The job application collects what an employment application normally does.

What we do with it

We use it to answer you, to arrange care, and — where you have signed an authorization — to apply for the Medicaid waiver on your behalf. We do not sell it, rent it, or share it for advertising.

Health information is shared only as your signed authorization allows: with the providers named on it, with the support coordination agency handling your case, and with the state agencies processing the application. You can withdraw that authorization at any time by writing to inquiries@healthcaresupporthc.com.

How long we keep it

Enquiries and questions are kept while they are useful for following up. Client records are kept for as long as Georgia licensing rules require, and are then destroyed securely. If you ask us to delete an enquiry that has not become a client record, we will.

Where it is stored

Information is stored in our secured systems, access-controlled so that only staff who need it can see it, with access logged. It is held on servers in the United States.

Cookies and tracking

This site does not use advertising cookies and does not track you across other websites. Your browser may store a small amount of information needed for pages to work, and our hosting provider keeps standard server logs.

Your choices

You can ask us what we hold about you, ask us to correct it, ask us to delete an enquiry, or withdraw an authorization. Write to inquiries@healthcaresupporthc.com or call (404) 375-7808 and we will deal with it.

Children

This website is intended for adults arranging care. We do not knowingly collect information from children through it.
